Overview

The Memory Care Manager oversees the personal care of residents with Alzheimer’s or other forms of dementia as well as programs that attend to psychosocial needs, self-expression and physical activity.   All efforts are targeted at promoting good health in an environment where the resident feels cared for, understood and accepted.

JOB DUTIES

 

Management of Resident Care Specialists

  • Interview, hire and ensure thorough new team member training for Care Specialists in collaboration with Health and Wellness Director
  • Conduct/ensure continuing education and training for Care Specialists
  • Implements and maintains a ‘Team Specific’ approach for Memory Care residents
  • Offers mentoring, coaching, counseling and disciplinary action of team as required

 

Management of Activity Program and Resident Engagement

  • Creates a well-balanced and appropriate resident activity calendar each month based on resident population and in collaboration with Director of Excitement
  • Searches for new and creative ways to engage residents in activity
  • Assigns Care Specialists, as appropriate, to certain activities on certain days
  • Monitors and documents resident participation in activities
  • Reports activity participation to families via photograph or conversation

 

Resident Care

  • Ensures resident care is occurring according to the care plan and in a quality, caring manner
  • Identifies resident (or team member) safety concerns and acts proactively to remedy through addressing the issue personally or reporting it appropriately
  • Ensures neat and tidy living space for residents both in suite and in common areas

 

Communication with Families

  • Works to stay in close contact with families about their loved one(s) but no less than the following:
    • Sends monthly activity calendars to families
    • Communicates with family members at least once monthly electronically or in person
    • Care Plan meetings as needed or at least every six (6) months
    • Holds ad hoc meetings as required and is flexible with families on scheduling

 

Administrative

  • Writes and posts monthly activity schedule in a timely manner
  • Writes and posts monthly Care team schedule no less than ten (10) days prior to month beginning
  • Write and manages Resident Care Plans (if a licensed nurse)
  • Maintains documentation related to team and Human Resources requirements
  • Conducts team member performance evaluations, routinely and as needed
  • Is familiar and ensures compliance with State regulatory requirements

 

Other

  • Works a full shift of hands on resident care monthly (alternating shifts)
  • Participates in the Manager On Call rotation
  • Maintains personal education to be current in memory impairment
  • Works to maintain proper infection control practices

Qualifications

  • At least six (6) months of experience with care of seniors in a healthcare environment is required
  • Current First Aide and CPR Certification required
  • Experience in the care of individuals with memory loss preferred
  • Certified by the National Council of Dementia Practitioners within six (6) months of start date if not already certified
  • Training or certifications in therapeutic recreation/activities preferred
  • If a nurse, a current, unencumbered license to practice nursing in this State
  • Management experience preferred
  • College degree preferred
